License Reactivation Fee $75.00 if license is expired for more than
1 year and all applicable renewal fees
Late License Fee $25.00 if received after the expiration date
License Verification Fee $15.00
Duplicate License Fee $10.00
Copy of roster of Alabama license massage therapists: thirty dollars ($30)
Copy of roster of approved massage therapy schools: five dollars ($5)
Copy fee per page: twenty five cents ($.25)
Postage and handling: actual postage plus one dollar fifty cents ($1.50) per item handling
Criminal History fee $25
All fees are nonrefundable
All fees must be submitted in the form of a cashier’s or certified
check, or money order. No personal checks or cash will be accepted.
Fees must be received at the Office of the Alabama Board of Massage Therapy on or before the proper due date,
and in the proper form of payment for applications or renewals to be considered timely filed.
A late fee of twenty-five dollars ($25.00) shall be assessed for all license renewal fees not post-marked
by the Board before the expiration date of the license.
